Help pay for up to 200 days of home healthcare, skilled nursing facility and
nursing home after you’ve been hospitalized.6
The ADA Extended Care Rider
Health conditions often require continued care after a hospitalization. What
if medical insurance doesn't cover all the expenses for extended care services, and
leaves you with a bill for expenses you may not have planned for?
The optional Extended Care Rider requires medical underwriting and can be added
to the ADA Hospital Indemnity Insurance Plan, to help pay for some of these long-term
care expenses. It provides a lump-sum cash payment for up to 200 days to help cover
home healthcare, skilled nursing, and nursing home expenses after a hospitalization.
This optional coverage is available for your spouse at the same rates that ADA members pay.
If you’re under age 70, you’ll receive:
- Home healthcare: up to $100 per day6
- Skilled nursing facility6 and nursing home: up to $250 per day
If you’re age 70 and over, you’ll receive:
- Home healthcare: up to $100 per day6
- Skilled nursing facility: up to $250 per day after 20 days6
- Nursing home: up to $250 per day after 90 days
ADA Hospital Indemnity Insurance Plan is available for your spouse and eligible dependents.7
You can provide your spouse and dependent children with the ADA Hospital Indemnity Insurance Plan.
Available to eligible ADA members’ spouses who are under the age of 65
Available to unmarried dependents under age 27

The Extended Care Rider provides a daily benefit for up to 200 days.
2Hospital Indemnity Insurance Plan claims are not payable for any confinement or treatment
resulting from a condition for which the insured received any medical treatment, care, advice, or medication
within 12 consecutive months before the effective date of this insurance and applicable to any insurance
increases. The pre-existing conditions limitation will apply for a maximum of 24 consecutive months after
the effective date of this insurance and any insurance increases.

4Beginning at age 65, benefit amounts can no longer be increased. Hospital Indemnity Insurance
Plan Coverage is renewable up to age 90.
